How does Stalevo 125mg+31.25mg+200mg Tablet work?

Stalevo 125mg+31.25mg+200mg Tablet combines three active ingredients:

• Levodopa: Converts to dopamine in the brain, a crucial neurotransmitter for controlling movement.
• Carbidopa: Prevents the breakdown of levodopa, allowing more of it to enter the brain.
• Entacapone: Prolongs the effect of levodopa by maintaining its level in the blood.

Together, these ingredients provide greater relief from Parkinson's disease symptoms.

Indication of Stalevo 125mg+31.25mg+200mg Tablet

Treatment of adult patients with Parkinson's disease experiencing end-of-dose motor fluctuations not stabilized on levodopa/DDC inhibitor treatment.

Pharmacology

• Levodopa: Converted to dopamine in the brain to alleviate Parkinson's symptoms.
• Carbidopa: Inhibits peripheral metabolism of levodopa, increasing its availability to the brain.
• Entacapone: Inhibits catechol-O-methyltransferase (COMT), enhancing and prolonging the effect of levodopa.

Dosage & Administration of Stalevo 125mg+31.25mg+200mg Tablet

Adults:

• The optimum daily dose must be determined by careful titration of levodopa in each patient.
• Patients should take only 1 tablet per dose administration.
• The maximum recommended daily dose of entacapone is 2,000 mg.
• This tablet is intended for patients currently treated with corresponding doses of standard-release levodopa/DDC inhibitor and entacapone.

Transferring Patients to Stalevo:

From Levodopa/DDC Inhibitor (Carbidopa or Benserazide) and Entacapone:

• Direct Transfer: Patients taking standard-release levodopa/carbidopa and entacapone can switch directly to corresponding Stalevo doses.
• Example: A patient taking 100 mg/25 mg levodopa/carbidopa and 200 mg entacapone four times daily can switch to Stalevo four times daily.
• Dosing Titration: Adjust Tridopa dosing carefully for optimal response, matching as closely as possible to the total daily levodopa dose.

From Levodopa/Benserazide and Entacapone:

• Discontinue levodopa/benserazide the night before starting Stalevo the next morning.
• Start with a dose that provides the same or slightly more levodopa (5-10%).

For Patients Not Currently Treated with Entacapone:

• Introduction: Consider initiating Stalevo at corresponding doses for patients with Parkinson's disease and end-of-dose motor fluctuations not stabilized on current standard release levodopa/DDC inhibitor treatment.
• Patients with Dyskinesia or High Levodopa Dose (>800 mg): Introduce entacapone separately and adjust the levodopa dose before switching to Stalevo.
• Dose Reduction: Reduce levodopa dose by 10-30% within the first days to weeks after starting Stalevo, particularly for patients with dyskinesia.

Dose Adjustment During Treatment:

• Increasing Levodopa: Increase the frequency of doses and/or use an alternative strength of Stalevo within recommended dose limits.
• Decreasing Levodopa: Reduce the total daily dose by decreasing the frequency or strength of Stalevo.
• Concomitant Use of Other Levodopa Products: Follow maximum dose recommendations for combined use with Stalevo.

Discontinuation of Stalevo:

• When discontinuing Stalevo and switching to levodopa/DDC inhibitor therapy without entacapone, adjust other antiparkinsonian treatments, especially levodopa, to maintain control of parkinsonian symptoms.

Interaction

• Drug-Drug Interactions: Inform your doctor if you are taking medications for depression, respiratory diseases, severe allergic reactions, heart diseases, high blood pressure, mental disorders, nausea, convulsions, muscle relaxation, or iron supplements.
• Drug-Food Interaction: Avoid taking with or shortly after eating protein-rich foods as it may decrease the absorption of the medicine.

Contraindications

Severe hepatic impairment, narrow-angle glaucoma, pheochromocytoma, MAO inhibitors, history of NMS, or hypersensitivity to components.

Pregnancy & Lactation

• Pregnancy: Category C; use only if the benefit justifies the risk.
• Lactation: Not recommended.
